NEWS YOU NEED TO KNOW

As we prepare for the upcoming plan year, please review the key updates to your benefits program outlined below. These changes will take effect on January 1, 2026. Community Care Partners (CCP) remains committed to offering a competitive and comprehensive suite of benefits to all team members. The following highlights provide important details about your 2026 benefits:

 

 

  • Dates: November 12th- November 26th

  • All Changes are effective January 1st, 2026

  • ACTIVE Enrollment This means if you do not participate you will NOT have benefits in 2026

  • Enrollment will need to be completed in the TakeCommand and Dayforce portal

 

  • Provided by: TakeCommand- ICHRA

  • Starting January 1, 2026, medical coverage will now be offered through Take Command.

  • Employees will have access to personalized plan options and a streamlined enrollment process through the Take Command platform. You may access the site at: https://www.takecommandhealth.com/community-care-partners

  • The first of four educational sessions will be held on Thursday, November 6th at 10:00 a.m. CT to guide employees through the new platform and enrollment process.

  • If you are enrolling in a significant other’s or spouse’s employer plan for 2026, you may request a Spousal Loss-of-Coverage Verification Notice from Community Care Partners. This document will confirm that CCP no longer offers employer-sponsored medical benefits and will assist with your new plan enrollment.

IMPORTANT INFORMATION: MEDICARE ENROLLMENT REQUIREMENTS

If you turn 65 during the upcoming plan year: Medicare enrollment is required, and you will need to make a mid-year election. Please contact the CCP Benefits Team at benefits@ccpartners.com for assistance in completing your CMS-L564 form.

 

Effective January 1, 2026 Community Care Partners will NO longer offer the following reimbursement accounts:

  • Health Flexible Spending Account (HCFSA)

  • Limited Purpose FSA (LPFSA)

  • Health Savings Account (HSA)

  • Dependent Care FSA (DCFSA)

 

Important Notes for Employees with HSA Accounts:

  • All existing HSA funds are yours to keep and manage.

  • You may continue to use these funds for eligible medical expenses.

  • You may open a new HSA through your bank or financial institution by providing proof of enrollment in a qualified High Deductible Health Plan (HDHP).
